Consent to Personal Data Processing

By filling in and submitting the enquiry form on the website https://babysmile.group, I hereby freely, of my own will and in my own interest, give my consent to Limited Liability Company “Bismile” (TIN (INN) 7805735106, KPP 780501001, OGRN 1187847307119, address: Office 304, Letter A, 15A Uryupin Lane, St Petersburg, 198095, Russia) (the “Operator”) to process my personal data on the terms set out below.

Personal data covered

• surname, first name and patronymic (or the name I entered in the form);
• contact phone number;
• email address;
• organisation and position — if I have provided them voluntarily;
• the content of my enquiry and my correspondence with the Operator;
• technical data: IP address, browser and device information, referral source and UTM tags, date and time of the enquiry.

Purposes of processing

• considering my enquiry and preparing a reply to it;
• corresponding with me on the substance of the enquiry, including by phone, email and messaging apps;
• routing the enquiry to the responsible employee of the Operator;
• keeping internal records of enquiries.

Actions performed with personal data

Collection, recording, systematisation, accumulation, storage, clarification (updating, modification), retrieval, use, blocking, deletion and destruction of personal data — with or without the use of automation.

Validity period and withdrawal of consent

This consent is valid until the purposes of processing are achieved or until it is withdrawn. I may withdraw my consent at any time by sending the Operator a notice in free form to info@bsmile.ru marked “Withdrawal of consent to personal data processing” or by post to: Office 304, Letter A, 15A Uryupin Lane, St Petersburg, 198095, Russia.

Upon receipt of the withdrawal, the Operator will stop processing my personal data and destroy it within no more than 30 days, unless processing must continue in accordance with the legislation of the Russian Federation.

I confirm that I have read the Operator’s Personal Data Processing Policy, that the data I have provided is accurate and that I am acting on my own behalf.
